Le m\u00e9dicament est destin\u00e9 \u00e0 \u00eatre inject\u00e9 par voie intra-articulaire dans l&rsquo;articulation du genou \u00e0 l&rsquo;aide d&rsquo;une aiguille st\u00e9rile de 21G x 2 pouces (0,8 x 50 mm). <\/p><p>La figure 1 montre la structure tridimensionnelle de l&rsquo;hydrogel de polyacrylamide. Lors de la synth\u00e8se, le N,N, m\u00e9thyl\u00e8ne-bis-acrylamide polym\u00e9rise avec l&rsquo;acrylamide, formant des liaisons transversales entre les cha\u00eenes de polyacrylamide. <\/p><p>L&rsquo;hydrogel de polyacrylamide r\u00e9ticul\u00e9 est synth\u00e9tis\u00e9 par un processus de polym\u00e9risation dans lequel des unit\u00e9s r\u00e9p\u00e9titives d&rsquo;acrylamide sont li\u00e9es entre elles par une r\u00e9action en cha\u00eene.<\/p><p>La r\u00e9action en cha\u00eene utilise un principe d&rsquo;initiateur redox, dans lequel un initiateur de persulfate d&rsquo;ammonium (AMPS) g\u00e9n\u00e8re un radical libre qui ajoute un monom\u00e8re d&rsquo;acrylamide en obtenant un \u00e9lectron de la double liaison dans l&rsquo;acrylamide, formant une seule liaison carbone-carbone avec l&rsquo;acrylamide. Cela laisse un \u00e9lectron non divis\u00e9 pour l&rsquo;ajout ult\u00e9rieur de monom\u00e8res d&rsquo;acrylamide, comme le montre la figure 2 \u00e0 droite. <\/p><p>La ramification de la structure carbone-carbone lin\u00e9aire du polyacrylamide se produit en raison de la pr\u00e9sence d&rsquo;un agent r\u00e9ticul\u00e9, le monom\u00e8re N,N-m\u00e9thyl\u00e8ne-bis-acrylamide (MBAM), comme le montre la figure 3.<\/p><p>Apr\u00e8s ramification des cha\u00eenes de polyacrylamide de la structure, un r\u00e9seau tridimensionnel de polyacrylamide crois\u00e9 est form\u00e9, au lieu de cha\u00eenes lin\u00e9aires de polyacrylamide non coupl\u00e9es.<\/p><p><sup>Arthrosamid\u00ae<\/sup> se pr\u00e9sente sous la forme d&rsquo;une seringue st\u00e9rile pr\u00e9remplie de 1 ml \u00e0 usage unique, munie d&rsquo;un raccord luer lock et d&rsquo;un capuchon d&rsquo;\u00e9tanch\u00e9it\u00e9. En r\u00e9sum\u00e9, en ce qui concerne l&rsquo;injection intra-articulaire, il a \u00e9t\u00e9 observ\u00e9 que les petites particules (8 \u00b5m ou moins) sont sujettes \u00e0 la phagocytose et s&rsquo;\u00e9coulent avec le liquide synovial \u00e0 travers les jonctions gap de l&rsquo;\u00e9pith\u00e9lium synovial et finissent par \u00eatre distribu\u00e9es de mani\u00e8re syst\u00e9mique, tandis que les particules plus grosses (&gt;8-17 \u00b5m) sont encapsul\u00e9es avec le temps et restent immobilis\u00e9es dans les tissus mous pour une dur\u00e9e ind\u00e9termin\u00e9e. Comme d\u00e9crit pr\u00e9c\u00e9demment, l&rsquo;<sup>Arthrosamid\u00ae\/<\/sup>\u00ab\u00a0hydrogel\u00a0\u00bb se pr\u00e9sente sous la forme d&rsquo;un r\u00e9seau r\u00e9ticul\u00e9 de cha\u00eenes de polyacrylamide. Le mat\u00e9riau final inject\u00e9 ne contient pratiquement pas de petites particules et la plus petite taille de particule mesurable a \u00e9t\u00e9 mesur\u00e9e \u00e0 &gt;300 \u03bcm, ce qui est bien sup\u00e9rieur \u00e0 la taille de particule d&rsquo;un micron signal\u00e9e comme \u00e9tant physiologiquement mobile.   <\/p><p>Dans des \u00e9tudes men\u00e9es sur des lapins et des chevaux, on a observ\u00e9 que des macrophages et des cellules g\u00e9antes \u00e9taient associ\u00e9s \u00e0 l&rsquo;hydrogel et qu&rsquo;il n&rsquo;y avait aucune preuve de la pr\u00e9sence de particules d&rsquo;hydrogel dans les phagosomes de ces cellules (Christensen et al., 2016). D&rsquo;autres \u00e9tudes \u00e9valuant les ganglions lymphatiques de drainage et les tissus \u00e9loign\u00e9s des sites d&rsquo;injection dans les tissus mous ont \u00e9t\u00e9 r\u00e9alis\u00e9es et n&rsquo;ont r\u00e9v\u00e9l\u00e9 aucune trace d&rsquo;hydrogel dans les ganglions lymphatiques de drainage locaux ou dans des organes plus \u00e9loign\u00e9s (Charles River, 2011). Chez les chevaux, l&rsquo;hydrogel est apparu 2 semaines apr\u00e8s le traitement sous la forme d&rsquo;une couche interne dans la couche synoviale, m\u00e9lang\u00e9e \u00e0 des cellules synoviales en multiplication, similaire \u00e0 l&rsquo;histologie observ\u00e9e dans un mod\u00e8le de lapin. Apr\u00e8s 1 mois, les cellules synoviales se sont apparemment d\u00e9plac\u00e9es vers la surface, et un sch\u00e9ma d&rsquo;absorption similaire a \u00e9t\u00e9 observ\u00e9 apr\u00e8s 3, 8 et 24 mois (Christensen et al., 2016). L&rsquo;hydrogel \u00e9tait pr\u00e9sent comme une zone int\u00e9gr\u00e9e dans l&rsquo;interstitium sous-synovial avec un fin r\u00e9seau de tissu vasculaire et tr\u00e8s peu de cellules inflammatoires.   <\/p><p>Une \u00e9tude histopathologique prospective a \u00e9t\u00e9 r\u00e9alis\u00e9e sur des tissus pr\u00e9lev\u00e9s sur des patients lors d&rsquo;une arthroplastie totale du genou (ATG). Les patients avaient \u00e9t\u00e9 trait\u00e9s avec de l&rsquo;hydrogel 5 \u00e0 33 mois auparavant. Un sch\u00e9ma histologique similaire a \u00e9t\u00e9 observ\u00e9 dans les sept cas : L&rsquo;hydrogel s&rsquo;est incorpor\u00e9 \u00e0 la membrane synoviale et les cellules du rev\u00eatement synovial externe ont envahi le gel et form\u00e9 une nouvelle couche de rev\u00eatement. Un sch\u00e9ma similaire a \u00e9galement \u00e9t\u00e9 d\u00e9crit 9 mois apr\u00e8s l&rsquo;ATG dans une \u00e9tude de cas (Christensen et Daugaard, 2016).   <\/p><p>L&rsquo;hydrogel de Contura est commercialis\u00e9 depuis 2001 et un certain nombre d&rsquo;essais cliniques avec des dur\u00e9es de suivi variables ont \u00e9t\u00e9 men\u00e9s pour diverses indications. Des donn\u00e9es \u00e0 long terme sur l&rsquo;hydrogel de polyacrylamide de Contura sont disponibles \u00e0 partir d&rsquo;une \u00e9tude de suivi de 10 ans portant sur 104 patients atteints du VIH auxquels on a inject\u00e9 en moyenne 6 ml d&rsquo;hydrogel <sup>Aquamid\u00ae<\/sup> pour le traitement de la lipoatrophie faciale. Lors du suivi (10 ans), aucun patient n&rsquo;a pr\u00e9sent\u00e9 de migration de l&rsquo;hydrogel et la majorit\u00e9 des patients \u00e9taient \u00ab\u00a0tr\u00e8s satisfaits\u00a0\u00bb (74,8 %) ou \u00ab\u00a0satisfaits\u00a0\u00bb (23,4 %) du r\u00e9sultat cosm\u00e9tique (Negredo et al., 2015).
